บทคัดย่อ/Abstract |
โรงพยาบาลส่งเสริมสุขภาพตำบล เป็นด่านหน้าการดำเนินงานสร้างเสริมสุขภาพที่ใกล้ชิดกับประชาชนในชุมชน การวิจัยแบบภาคตัดขวางเชิงวิเคราะห์ มีวัตถุประสงค์เพื่อศึกษาระดับและปัจจัยที่ส่งผลต่อความสามารถในการสร้างเสริมสุขภาพของเจ้าหน้าที่สาธารณสุขโรงพยาบาลส่งเสริมสุขภาพตำบล จังหวัดปัตตานี เก็บข้อมูลจากกลุ่มตัวอย่างเจ้าหน้าที่สาธารณสุข จำนวน 118 คน สุ่มตัวอย่างแบบหลายขั้นตอน เก็บข้อมูลโดยใช้แบบสอบถาม ใช้สถิติเชิงพรรณนา ได้แก่ ความถี่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และสถิติการวิเคราะห์ถดถอยเชิงพหุแบบถอยหลัง ผลการศึกษา พบว่า เจ้าหน้าที่สาธารณสุขมีระดับความสามารถในการสร้างเสริมสุขภาพตามกลยุทธ์สร้างเสริมสุขภาพกฎบัตรออตตาวา ภาพรวม 3 ด้านอยู่ในระดับมาก (x? = 3.92, S.D. = 0.47) รายด้าน พบว่า การไกล่เกลี่ย การประสานงาน มีค่าเฉลี่ยสูงสุด (x? = 3.99, S.D. = 0.54) รองลงมา คือ การสนับสนุนชี้แนะ (x? = 3.93, S.D. = 0.53) และการเสริมสร้างความสามารถ (x? = 3.81, S.D. = 0.53) ตามลำดับ และ พบว่าปัจจัยการบริหารด้านการจัดการ (? = 0.559) และ ด้านงบประมาณ (? = 0.163) ส่งผลอย่างมีนัยสำคัญทางสถิติต่อความสามารถในการสร้างเสริมสุขภาพของเจ้าหน้าที่สาธารณสุข (p < 0.001 และ p = 0.049 ตามลำดับ) การรับรู้สมรรถนะแห่งตนในการปฏิบัติงานในพื้นที่เหตุการณ์ความไม่สงบ (? = 0.076, p = 0.288) เป็นปัจจัยร่วมในการทำนาย ปัจจัยทั้งสามสามารถร่วมทำนาย ร้อยละ 45.3 <br /><br />
หน่วยงานสาธารณสุขควรพัฒนาศักยภาพเจ้าหน้าที่ ด้านทักษะการสื่อสารสาธารณะ การคืนข้อมูล การทำนโยบายสาธารณะ และการประสานงานกับองค์กรภาครัฐในชุมชน ให้ความสำคัญกับการใช้วงจรการบริหารงานคุณภาพของเดมมิ่ง Plan Do Check Act (PDCA) ในการจัดการที่มีคุณภาพ และต่อเนื่อง การสนับสนุนงบประมาณที่เพียงพอต่อการใช้จ่ายค่าตอบแทนต่างๆ<br /><br />
Sub - district health promoting hospital is the frontline of working on health promotion and it is also very close to population in a community. This cross sectional analytical study aimed to investigate ability levels and factors affecting abilities on health promotion of health officers in sub ? district health promoting hospitals in Pattani Province. One hundred and eighteen samples were selected using multistage random sampling. Data were collected using questionnaires. Descriptive statistics: frequency, percent, mean and standard deviation, and backward multiple linear regression were performed for data analysis. The results found that health officers had a high level of abilities on health promotion following the three main strategies of the Ottawa Charter (x? = 3.92, S.D. = 0.47). For each strategy, mediate character was at the highest level (x? = 3.99, S.D. = 0.54), followed by advocate (x? = 3.93, S.D. = 0.53) and enable (x? = 3.81, S.D. = 0.53), respectively. In multiple analyses, managerial factor (? = 0.559) and budgetary factor (? = 0.163) were statistically significantly affected on abilities in health promotion of health officers (p < 0.001 and p = 0.049, respectively). The self-efficacy on working in the unrest areas (? = 0.076, p = 0.228) was a covariate reserved in the final regression model. The above three factors could predict on abilities in health promotion of the health officers by 45.3%. <br /><br />
Concerning public health organizations should develop abilities of health officers in health promotion emphasizing on the skills of public communication, data feedback, public policy building and coordinating with governmental sectors in communities. The administrative cycle by Deming: plan, do, check, and act (PDCA) should be considerably used in managements qualitatively and continuously. Also, sufficiently support budgets for compensation of health officers should be considered in administration.<br /><br />
